Localized hyperhidrosis in pretibial myxedema.

Summary

Localized sweating (hyperhidrosis) in pretibial myxedema lesions was observed. Sweat glands in affected skin were larger and produced more sweat, a previously unreported finding.

Background:

Pretibial myxedema is a skin manifestation of autoimmune thyroid disease.
Associated symptoms typically include skin thickening and edema.
Hyperhidrosis is not a commonly recognized feature of pretibial myxedema.

Observation:

Two patients presented with spontaneous excessive sweating (hyperhidrosis) exclusively on their pretibial myxedema lesions.
Quantitative sweat measurements after methacholine stimulation showed a 2-4 fold increase in sweat rate in lesional skin compared to adjacent normal skin.
Histological examination of biopsy specimens revealed significantly enlarged eccrine secretory glands within the pretibial myxedema lesions.

Findings:

This study reports the novel finding of localized hyperhidrosis associated with pretibial myxedema.
The increased sweat production is linked to hypertrophy of eccrine glands in the affected skin.
This localized hyperhidrosis appears to be a specific phenomenon within pretibial myxedema.

Implications:

Understanding this association may aid in diagnosing and managing thyroid-related dermatopathies.
Further research is warranted to elucidate the pathophysiology linking autoimmune processes to eccrine gland function.
This finding could lead to new therapeutic strategies targeting sweat gland abnormalities in autoimmune skin conditions.
