This study evaluated an integrated pharmaceutical care programme for patients with chronic diseases like heart failure and COPD. Over 12 months, 238 patients were followed, with quality of life measured at the start and end. The study found that medication-related problems were common, with most being avoidable and resolved by pharmacists. However, no significant improvements in readmissions, emergency visits, or quality of life were observed. The authors suggest that electronic systems are needed to improve communication between healthcare providers. Overall, the programme may help improve medication safety but did not lead to major changes in health outcomes or costs.
