Summary

Internal root resorption is common in teeth with pulp inflammation or necrosis, contrary to previous assumptions. This study found no resorption in healthy teeth, but significant instances in those with pulpitis or necrosis.

Background:

  • Internal inflammatory root resorption is considered rare, often undetected clinically.
  • Pulp inflammation is a suspected etiological factor in internal resorption.
  • Previous studies lacked sufficient data on the prevalence of internal resorption in relation to pulp vitality.

Purpose of the Study:

  • To test the hypothesis that there is no difference in internal resorption between healthy vital pulps and those with a history of inflammation.
  • To investigate the prevalence of internal resorption in teeth with varying pulpal statuses.

Main Methods:

  • Thirty human teeth without prior root canal treatment were analyzed.
  • Pulpal status was diagnosed (healthy, pulpitis, necrosis) before extraction.
  • Scanning electron microscopy was used to examine root canal walls for internal resorption after organic debris removal.

Main Results:

  • Internal resorption was absent in all healthy pulp teeth (0/9).
  • Internal resorption was found in 50% of teeth with pulpitis (4/8) and 77% of teeth with necrosis (10/13).
  • Resorptive lesions were shallow (<100 μm deep) and most prevalent in the middle and apical thirds of the root.

Conclusions:

  • Internal resorption is a frequent finding in teeth with pulp inflammation or necrosis.
  • The study refutes the notion that internal resorption is rare.
  • Pulp inflammation and necrosis are strongly associated with internal root resorption.
