Kidney Transplant II: Surgical Procedure01:26

Kidney Transplant II: Surgical Procedure

Preoperative ManagementThe primary goals of preoperative management in kidney transplantation are to optimize the patient’s metabolic state and prepare them for surgery through diet adjustments, necessary dialysis, and tailored medical treatment. This phase also involves comprehensive infection screening and patient education about the surgical procedure and postoperative care to improve outcomes and adherence.Medical ManagementA comprehensive evaluation is required for both the living donor...

Urinary Tract Calculi VI: Surgical Management

Procedures for Kidney StonesMedical intervention is necessary when kidney stones or renal calculi are too large to pass spontaneously (typically greater than 5 millimeters) when stones are accompanied by symptomatic infection (such as fever or pyelonephritis), when they impair kidney function, or when they cause persistent symptoms like severe pain, nausea, or urinary retention. Additionally, patients with only one kidney or those who cannot be treated with medical management also require...

Pharmacokinetics in Geriatric Patients: Effect of Age on Drug Excretion

In geriatric patients, renal physiology undergoes significant changes, including diminished renal blood flow and a lower glomerular filtration rate (GFR), leading to alterations in medication clearance. Drugs such as aminoglycoside antibiotics, lithium, and digoxin, which rely on glomerular filtration for removal from the body, particularly impact pharmacokinetics. These drugs tend to have slower clearance rates in older adults, necessitating careful dosage considerations.Evaluation of renal...

Summary

Laparoscopic renal surgery is safe and effective for elderly patients, showing comparable outcomes to younger patients. Age should not prevent older individuals from receiving this minimally invasive treatment.

Area of Science:

  • Urology
  • Minimally Invasive Surgery
  • Geriatric Surgery

Background:

  • Laparoscopic renal surgery offers advantages over open procedures.
  • Data on laparoscopic surgery benefits in elderly patients are limited.

Purpose of the Study:

  • Compare perioperative outcomes of laparoscopic renal surgery in patients aged 70+ versus under 70.
  • Evaluate the E-PASS (estimation of physiologic ability and surgical stress) scoring system.

Main Methods:

  • Retrospective study of 55 patients undergoing laparoscopic nephrectomy or nephroureterectomy (Jan 2006-Dec 2009).
  • Comparison of perioperative parameters including surgical stress score (SSS), complications, operation time, and hospital stay.
  • Assessment of the E-PASS system for risk prediction.

Main Results:

  • Perioperative parameters were comparable between elderly (70+) and younger patients.
  • Low perioperative morbidity and surgical stress observed in elderly patients.
  • Laparoscopic renal surgery demonstrated feasibility and good tolerance in the geriatric population.

Conclusions:

  • Laparoscopic renal surgery is a viable and well-tolerated option for elderly patients.
  • Age alone should not be a contraindication for definitive laparoscopic renal surgery.
  • Further validation of the E-PASS scoring system in urologic surgery is recommended.
