Imaging findings in head and neck Kimura disease (KD) show multiple ill-defined enhancing masses, often around the parotid gland. Associated lymphadenopathy is common in this rare inflammatory disorder.

Purpose of the Study:
- To evaluate the characteristic computed tomography (CT) and magnetic resonance (MR) imaging findings of Kimura disease in the head and neck.
- To correlate imaging findings with histopathological confirmation.

- Retrospective review of CT (n=21) and MR (n=9) imaging studies in 28 patients with histologically proven head and neck KD.
- Analysis of lesion characteristics including location, size, margins, signal intensity, enhancement patterns, and associated findings like lymphadenopathy.
Main Results:
- A total of 52 non-nodal lesions were identified, predominantly unilateral or bilateral masses.
- The parotid and/or periparotid regions were the most common sites (36 lesions in 23 patients).
- Lesions typically presented as ill-defined, enhancing masses with iso- to hyperintense signal on T1-weighted and hyperintense signal on T2-weighted MR images, often with associated lymphadenopathy.
Conclusions:
- Multiple, ill-defined, enhancing masses within and around the parotid gland are characteristic CT and MR imaging findings of head and neck Kimura disease.
- Associated regional lymphadenopathy further supports the diagnosis.
- Imaging plays a crucial role in the diagnosis and characterization of head and neck KD.
