Two gallbladder cancer cases with solitary lymph node metastasis showed no recurrence after radical surgery. This suggests a potentially good prognosis for solitary node-positive gallbladder carcinoma via specific lymphatic routes.

Area of Science:

  • Surgical Oncology
  • Gastrointestinal Pathology
  • Medical Imaging

Background:

  • Gallbladder cancer is often diagnosed at advanced stages.
  • Early detection and understanding lymphatic spread are crucial for effective treatment.

Observation:

  • Two male patients (85 and 73 years old) presented with gallbladder tumors.
  • Both underwent radical surgery with gallbladder-bed resection and lymphadenectomy.
  • Pathological examination revealed adenocarcinoma with solitary lymph node metastasis.

Findings:

  • Both patients remained recurrence-free for over 15 and 22 months post-surgery.
  • Metastasis was solitary and located along the right side of the hepatoduodenal ligament.
  • This route is one of two suggested lymphatic drainage pathways for gallbladder cancer.

Implications:

  • Solitary lymph node metastasis in gallbladder cancer may indicate a favorable prognosis.
  • Further studies with longer follow-up or larger cohorts are needed to confirm these findings.
  • Understanding specific lymphatic drainage patterns can refine surgical strategies and patient outcomes.
