This study presents a rare case of low-grade, nonintestinal sinonasal adenocarcinoma in an oil painter. The classification of sinonasal adenocarcinoma is crucial for predicting patient prognosis.

- Immunohistochemical analysis showed tumor cells positive for CK7, focally positive for CK20, and negative for CDX2, consistent with a nonintestinal type.

- The tumor exhibited features of a low-grade nonintestinal type, with specific immunohistochemical markers supporting this classification.
- Negative results for S-100, Actin, p63, TTF-1, and p53, along with a low Ki-67 index (2%), further characterized the tumor.
- The distinction between intestinal and nonintestinal types is significant, as intestinal types are typically high-grade, while nonintestinal types are generally low-grade.
