Area of Science:

  • Hepatology
  • Genetics
  • Internal Medicine

Background:

  • Wilson's disease is an inherited disorder of copper metabolism.
  • Fulminant hepatic failure (FHF) is a rare but severe presentation of Wilson's disease.

Observation:

  • A 21-year-old female presented with FHF and hemolysis.
  • Clinical diagnosis was supported by low ceruloplasmin and serum copper levels.
  • Postmortem examination confirmed Wilson's disease with hepatic copper accumulation.

Findings:

  • The patient's siblings showed varied carrier and disease statuses.
  • Wilson's disease presenting as FHF with hemolysis is associated with a poor prognosis.

Implications:

  • Wilson's disease should be suspected in cases of FHF with hemolysis.
  • Prompt diagnosis and consideration for early liver transplantation are vital for improving outcomes.
