Background:

Cesarean sections are increasingly common globally.
Operative complications can arise, including rare genitourinary fistulae.

Observation:

A 27-year-old woman presented with pelvic pain and dysuria post-cesarean section.
Microscopic hematuria was noted on urinalysis.
Transvaginal ultrasonography and hysterography confirmed a vesicouterine fistula.

Findings:

The patient had a history of two prior cesarean deliveries.
A vesicouterine fistula was successfully diagnosed and surgically repaired via a transperitoneal approach.
The surgical intervention was performed without complications.

Implications:

This case underscores the potential for severe maternal complications, such as genitourinary fistulae, following cesarean delivery.
Rising cesarean rates necessitate heightened awareness and vigilance for operative complications.
Improved surgical techniques and patient selection are crucial for minimizing risks.
