Summary

Gene expression profiling of dopamine receptor D1 (DRD1) and FAM71F2 in testicular seminoma can predict metastasis. This gene expression analysis offers improved decision-making for early adjuvant therapy or increased surveillance.

Area of Science:

  • Oncology
  • Molecular Biology
  • Genetics

Background:

  • Testis cancer treatment and staging rely on histological subtype and clinical stage.
  • Accurate staging is crucial, but CT scans cannot detect occult metastases.
  • Current predictive parameters for metastasis, like tumor size and vessel invasion, lack consistent validation.

Purpose of the Study:

  • To evaluate gene expression profiling for predicting metastatic status in testicular seminoma.
  • To compare the predictive accuracy of gene expression with established clinical and pathological parameters.

Main Methods:

  • Total RNA was isolated from testicular tumors of metastasized (n=12) and non-metastasized (n=40) patients, and normal tissue (n=36).
  • Real-time quantitative polymerase chain reaction (qPCR) was used to analyze gene expression of 94 candidate genes.

  • Logistic regression analysis was performed on normalized gene expression data and histological variables.
  • Main Results:

    • Expression of dopamine receptor D1 (DRD1) and FAM71F2 significantly discriminated metastasis status (P=0.004 for combined genes).
    • Combined gene expression achieved 87.7% concordance in predicting metastasis, outperforming primary tumor size (61% concordance).
    • Combining tumor size with gene expression did not improve predictive accuracy.

    Conclusions:

    • Gene expression analysis of DRD1 and FAM71F2 can accurately predict metastasis in testicular seminoma.
    • This approach offers improved individual decision-making for adjuvant therapy or surveillance strategies.
    • Gene expression profiling presents a promising tool for enhancing testicular cancer staging and management.
