Background:

  • Posterior capsule opacification (PCO) is a common complication after cataract surgery.
  • Predicting PCO can help optimize patient management and reduce the need for secondary interventions like laser capsulotomy.

Purpose of the Study:

  • To develop and evaluate artificial intelligence (AI) models for predicting the occurrence of PCO after phacoemulsification.
  • To assess the performance of AI models against traditional statistical methods like logistic regression.

Main Methods:

  • A clinical-based cross-sectional study was conducted.
  • Ten input variables were selected, and a decision tree (QUEST algorithm) and three back-propagation artificial neural networks (ANNs) with varying neurons (4, 20, 40) were constructed.
  • Models were trained on 282 eyes and tested on 70 eyes, comparing performance metrics with logistic regression.

Main Results:

  • The best performing ANN, with 40 neurons, achieved 87% accuracy and a positive likelihood ratio of 8 in predicting PCO requiring laser capsulotomy.
  • The area under the receiver-operating-characteristic curve for the best ANN was 0.71.
  • Logistic regression achieved 80% accuracy but had zero sensitivity, rendering its likelihood ratio immeasurable.

Conclusions:

  • A prototype artificial neural network demonstrated reasonable accuracy in predicting posterior capsule status requiring capsulotomy.
  • AI models show promise for improving the prediction of PCO post-cataract surgery.
