Summary

A rare case of trigeminal neuralgia linked to a persistent trigeminal artery (PTA) and a temporal cavernoma is presented. Managing hypertension led to spontaneous pain relief, with surgery planned for the cavernoma.

Background:

  • Trigeminal neuralgia (TN) is a debilitating neuropathic pain condition.
  • Persistent trigeminal artery (PTA) is a rare vascular anomaly where the fetal connection between the internal carotid and basilar arteries persists.
  • Cavernomas are congenital vascular malformations that can cause neurological symptoms.

Observation:

  • A patient presented with typical trigeminal neuralgia affecting the second and third divisions of the right facial nerve.
  • Symptoms were exacerbated by physical exertion and associated with unstable hypertension.
  • Magnetic Resonance Imaging (MRI) and Angio-MRI revealed a persistent carotid basilar anastomosis (PTA) and an asymptomatic left temporal cavernoma.

Findings:

  • The patient experienced spontaneous resolution of trigeminal neuralgia symptoms after medical management of hypertension.
  • The underlying cause was attributed to the persistent trigeminal artery compressing the trigeminal nerve.
  • A left temporal cavernoma was incidentally discovered and deemed a potential future concern.

Implications:

  • This case highlights the potential neurovascular compression role of PTA in trigeminal neuralgia.
  • Effective management of associated conditions like hypertension may lead to symptom improvement.
  • Surgical intervention for the cavernoma is planned, underscoring the importance of addressing all contributing factors in complex neurological cases.
