Background:

Vitreous cysts are rare ocular conditions.
Early diagnosis and monitoring are crucial for visual preservation.

Observation:

A 12-year-old presented with a 6-month history of visual disturbances in the left eye.
Funduscopy revealed a free-floating cyst in the anterior vitreous.
Magnetic resonance imaging (MRI) and B-scan ultrasound confirmed a hypoechogenic cyst (3.2 × 4.3 mm).

Findings:

Ultrasound biomicroscopy (UBM) showed a normal ciliary body, suggesting the cyst's origin was not related to ciliary body pathology.
The patient maintained good visual acuity (VA) of 1/0.9 despite the cyst's presence.

Implications:

Periodic monitoring is recommended due to the patient's good visual acuity.
Intervention with Nd:YAG or Argon laser photocystotomy, or pars plana vitrectomy, is considered if the cyst obstructs the visual axis.
