Summary

Technical errors in patellar resurfacing during total knee arthroplasty (TKA) can cause complications. A standardized technique yielded excellent clinical outcomes, though minor alignment errors persisted in some patients.

Background:

  • Patellar resurfacing errors in total knee arthroplasty (TKA) are linked to patellofemoral joint complications.
  • These complications include instability, patellar fracture, and wear of metal-backed patellar implants.

Purpose of the Study:

  • To evaluate the accuracy of a standardized patellar resurfacing technique during TKA.
  • To assess clinical outcomes and radiographical results following this technique.

Main Methods:

  • A review of 50 TKAs utilizing a condylar prosthesis and a standardized patellar resurfacing technique.
  • Patient evaluation included clinical scores (Hospital for Special Surgery Knee Score, Knee Society Knee Score) and radiographic assessment at a mean of 2.5 years post-surgery.

Main Results:

  • Significant improvements were observed in both Hospital for Special Surgery Knee Scores (mean 56 to 92) and Knee Society Knee Scores (pain 28 to 96, function 49 to 85).
  • 92% of patients achieved excellent and 8% good clinical results, with no patellofemoral symptoms, fractures, dislocations, or loosening.
  • Radiographs showed minor errors in 9 patellae (asymmetrical resurfacing) and 5 patellae (tilted), but no patellar subluxations.

Conclusions:

  • A standardized technique for patellar resurfacing in TKA can achieve satisfactory clinical results.
  • Despite good outcomes, minor technical errors in resurfacing and alignment remain a possibility.
