Summary

Burr hole surgery for chronic subdural hematoma (CSDH) with closed system drainage showed a trend towards better neurological recovery and lower recurrence rates compared to surgery without drainage in Thai patients.

Background:

  • Chronic subdural hematoma (CSDH) is a common neurosurgical condition.
  • Treatment options for CSDH include burr hole evacuation, with or without closed system drainage.

Purpose of the Study:

  • To compare the outcomes of burr hole treatment for CSDH with and without closed system drainage.
  • To evaluate recurrence rates and neurological recovery in Thai patients.

Main Methods:

  • Retrospective study of 143 Thai patients treated for CSDH at Ramathibodi Hospital (2002-2008).
  • Patients underwent burr hole surgery with or without closed system drainage.
  • Outcomes assessed via post-operative CT/MRI, complications, recovery, mortality, and recurrence.

Main Results:

  • 97 patients received drainage; 46 did not. No significant difference in clinical profiles.
  • CSDH recurrence was observed in 16% of the drainage group and 26% of the no-drainage group.
  • Coagulopathy may influence recurrence, but other clinical profiles showed no correlation.

Conclusions:

  • The closed system drainage group exhibited a higher rate of complete neurological recovery.
  • A lower recurrence rate of CSDH was noted in the drainage group.
  • Statistical significance was not reached for observed differences in recovery or recurrence.
