Observation:

  • A 69-year-old male presented with chest tightness and dyspnea.
  • Imaging revealed pericardial effusion and a left ventricular cavity.
  • Differential diagnoses included pseudo-pseudoaneurysm and myocardial abscess.

Findings:

  • The patient had a history of aortic valve replacement and mitral annuloplasty.
  • Echocardiography and CT demonstrated a 16-mm cardiac cavity.
  • MRI suggested pseudo-pseudoaneurysm or abscess.

Implications:

  • Successful surgical repair of myocardial defects is achievable.
  • This case highlights the importance of considering rare complications after cardiac surgery.
  • Vascular graft patching offers a viable solution for myocardial defect repair.
