Area of Science:

  • Urology
  • Surgical Oncology
  • Minimally Invasive Surgery

Background:

  • Limited data exists on the learning curve for laparoendoscopic single-site (LESS) surgery.
  • Evaluating the learning curve is crucial for adopting new surgical techniques.

Purpose of the Study:

  • To assess the learning curve associated with LESS radical nephrectomy for localized renal-cell carcinoma (RCC).
  • To identify specific surgical steps influencing the learning curve in LESS radical nephrectomy.

Main Methods:

  • Analysis of the first 50 consecutive LESS radical nephrectomy cases.
  • Video review to assess three core surgical components: medial colon retraction, hilar dissection, and kidney mobilization.
  • Correlation of operative times for each component with surgeon experience.

Published on: August 15, 2025

Main Results:

  • Operative time decreased significantly within the first 15 cases and stabilized thereafter.
  • Perioperative complication rates were comparable across patient groups, though slightly higher in the initial 30 cases.
  • Surgeon experience strongly correlated with reduced operative times for colon retraction and kidney mobilization.

Conclusions:

  • LESS radical nephrectomy for localized RCC is safe and effective, requiring a brief learning period for experienced laparoscopic surgeons.
  • Kidney mobilization emerged as the surgical step most impacted by the learning curve during the initial phase of LESS radical nephrectomy.
