Purpose of the Study:

  • To assess the performance of visual inspection with acetic acid (VIA), human papillomavirus (HPV) DNA testing, and cervical cytology.
  • To compare sequential and parallel combinations of these tests for optimal cervical cancer screening.
  • To determine the most effective screening strategy for symptomatic women.

Main Methods:

  • Conventional cytology, HPV DNA testing (HC2 assay), and VIA were performed on 548 symptomatic women.
  • Self- and physician-collected samples were used for HPV testing.
  • Colposcopy with biopsy served as the reference standard for all participants.

Main Results:

  • The sensitivity and specificity varied by test: VIA (82.5%, 66.9%), Pap (≥ASCUS) (77.5%, 86.8%), HPV-Physician (90.0%, 91.5%), HPV-Self (80.0%, 88.1%).
  • Sequential testing demonstrated superior performance compared to parallel testing.
  • VIA followed by HPV testing achieved the highest specificity (90.4%) and diagnostic accuracy (90.5%).

Conclusions:

  • A sequential VIA and HPV testing strategy is economically viable and highly accurate for cervical cancer screening.
  • This approach reduces referral rates, treatment burdens, and the number of patient visits.
  • For high-resource settings, HPV testing followed by reflex Pap cytology is a viable alternative.
