Area of Science:

  • Hematology
  • Surgical Hemostasis

Background:

  • Desmopressin (DDAVP) is a synthetic analog of vasopressin used to treat bleeding disorders.
  • Antifibrinolytic agents are used to prevent clot breakdown.

Purpose of the Study:

  • To evaluate the efficacy and safety of desmopressin as a hemostatic agent in surgical patients.
  • To review the applications and safety of antifibrinolytic therapy in surgical settings.

Main Methods:

  • Review of existing literature on desmopressin use in hemophilia, von Willebrand disease, and uremic patients.
  • Analysis of studies on antifibrinolytic therapy in cardiac, urological, and other surgical procedures.

Main Results:

  • Desmopressin demonstrates safety and efficacy in mild to moderate hemophilia and von Willebrand disease, requiring pre-surgical trials due to variable patient response.
  • Uremic patients benefit from desmopressin over cryoprecipitate for bleeding control.
  • Antifibrinolytic therapy reduces bleeding post-cardiac surgery without increased thrombotic risk.
  • Antifibrinolytics have expanded applications in neurosurgery, oral surgery, and managing gastrointestinal bleeding.

Conclusions:

  • Desmopressin is a valuable hemostatic option for specific surgical patient groups, emphasizing individualized treatment.
  • Antifibrinolytic therapy is safe and effective for reducing surgical bleeding in diverse clinical scenarios.
