Summary

Inflammatory abdominal aortic aneurysms can cause severe pain and hydronephrosis. Surgical aortic replacement and ureterolysis for hydronephrosis offer excellent outcomes and pain relief.

Background:

  • Inflammatory abdominal aortic aneurysm (IAAA) is a distinct subtype characterized by a thick, fibrous layer.
  • IAAA can present with significant abdominal or back pain and may cause urinary tract complications.

Purpose of the Study:

  • To analyze the clinical presentation, diagnosis, and surgical management of inflammatory abdominal aortic aneurysms.
  • To evaluate the outcomes of aortic replacement and ureterolysis in patients with IAAA.

Main Methods:

  • Retrospective review of 517 abdominal aortic aneurysm operations performed between 1971 and 1988.
  • Analysis of preoperative diagnosis, surgical interventions (aortic replacement, ureterolysis), and patient outcomes.

Main Results:

  • 45 cases (8.7%) of IAAA were identified.
  • Two-thirds of patients experienced severe pain; 20% developed hydronephrosis due to ureteral compression.
  • Preoperative diagnosis was infrequent (10 cases). Ureterolysis in 9 patients with hydronephrosis led to complete resolution.

Conclusions:

  • Aortic replacement is crucial for preventing rupture and relieving pain in IAAA patients.
  • Ureterolysis is a safe and effective procedure for resolving hydronephrosis caused by IAAA.
