An Improved Mechanical Testing Method to Assess Bone-implant Anchorage

Background:

  • Interchangeable abutments offer flexibility in dental restorations.
  • However, variations in material composition and manufacturing can affect mechanical stability.
  • Abutment screw loosening is a critical concern for long-term implant success.

Purpose of the Study:

  • To evaluate and compare the screw loosening of interchangeable abutments connected to internal connection implants.
  • To assess the mechanical integrity of different abutment systems under cyclic loading.

Main Methods:

  • Four groups of abutments (Straumann, Restore COC, Neoplant, AVANA) were tested with Straumann implants (n=7 each).
  • Abutments were tightened to 35 Ncm, subjected to 1 million cycles of 150 N at a 30-degree angle.
  • Periotest values (PTVs) and removal torque values (RTVs) were measured before and after loading.

Main Results:

  • The Straumann solid abutment and Restore COC abutment showed no mechanical failures.
  • Neoplant abutments experienced 86% screw fractures, and AVANA abutments had 57% implant fractures.
  • Straumann abutments exhibited significantly higher RTVs and lower final PTVs, indicating greater stability.

Conclusions:

  • Despite interchangeability, abutments from different manufacturers have varying physical and chemical properties.
  • Using implants and abutments from the same brand is recommended to prevent abutment screw loosening.
  • This ensures optimal mechanical interlocking and long-term stability of the restoration.
