Summary

Rheumatoid arthritis (RA) patients under 40 showed normal sympathoneural and adrenomedullary system responses during orthostatic challenges. This suggests these systems are not impaired in younger RA individuals.

Background:

  • The sympathoneural and adrenomedullary systems play a role in immune regulation.
  • Impairment of these systems has been hypothesized in rheumatoid arthritis (RA).

Purpose of the Study:

  • To evaluate sympathetic response to orthostasis in young female RA patients.
  • To compare autonomic nervous system activity between RA patients and healthy controls.

Main Methods:

  • Assessed plasma epinephrine and norepinephrine levels.
  • Analyzed heart rate variability (HRV) for sympathoneural activity.
  • Measured blood pressure and electrocardiogram during orthostatic testing in 22 RA females (<40 years) and 15 controls.

Main Results:

  • RA patients and controls exhibited similar plasma epinephrine and norepinephrine levels.
  • Blood pressure response to orthostasis was comparable between groups.
  • Heart rate and HRV parameters did not differ between RA patients and controls.

Conclusions:

  • Younger RA patients (<40 years) demonstrate normal sympathoneural and adrenomedullary system reactivity during orthostatic challenges.
  • Autonomic nervous system function appears preserved in this demographic of RA patients.
