Abstract:
Dental laboratory work authorization forms embrace a diversity of material content and design. This survey examined 51 dental laboratory work authorization forms and delineated each content item and format features into a frequency percentage. This information has a high potential for use in changing prospective dental laboratory and authorization form design.
