[Comparative value of electrocardiographic criteria of myocardial ischemia in exercise tests]
Insights
Depressed S-T segments during exercise tests are the most reliable indicator for diagnosing coronary heart disease (CHD). Other signs like T-wave inversion are less significant for accurate diagnosis and severity assessment.
Area of Science:
- Cardiology
- Diagnostic Imaging
- Exercise Physiology
Background:
- Coronary heart disease (CHD) is a leading cause of mortality worldwide.
- Accurate diagnosis and severity assessment of CHD are crucial for effective patient management.
- Exercise testing is a common method for evaluating myocardial ischemia in patients with suspected CHD.
Purpose of the Study:
- To evaluate the diagnostic reliability of various electrocardiographic (ECG) criteria during exercise testing for coronary heart disease.
- To determine the most significant indicators of myocardial ischemia in patients with confirmed CHD.
Main Methods:
- A comparative study involving 108 patients with confirmed coronary heart disease and 51 healthy controls.
- Performance of treadmill and isometric exercise tests.
- Analysis of various electrocardiographic parameters, including S-T segment changes, T-wave abnormalities, and chest pain.
Main Results:
- Depressed S-T segments demonstrated the highest diagnostic reliability for myocardial ischemia ( >90% probability).
- T-wave inversion and wider R-wave amplitude were moderately reliable (approx. 70% probability).
- Other criteria, such as S-T segment elevation/depression and abnormal T-wave amplitude, showed lower diagnostic value ( <50% probability).
Conclusions:
- Exercise-induced S-T segment depression is a highly reliable criterion for diagnosing coronary heart disease.
- Comprehensive assessment using multiple exercise test parameters is recommended for accurate CHD diagnosis and severity evaluation.
- Further research may refine the interpretation of exercise ECG criteria for improved clinical decision-making.
Abstract:
Treadmill and isometric tests were carried out in 108 patients with coronary heart disease confirmed at coronary arteriography versus 51 healthy subjects. Among myocardial ischemia criteria depressed S-T segment came out as most reliable (probability of valid diagnosis greater than 90%), less significant was wave T inversion, wider wave R amplitude, painful syndrome (probability 70%). The rise and depression of oblique upward segment S-T, abnormal T amplitude proved inferior to the above signs (probability less than 50%). To achieve valid diagnosis of coronary heart disease and to assess its severity, exercise tests should produce reliable criteria of myocardial ischemia.
