Summary
Cecal carcinoma, a common large bowel malignancy, infrequently causes obstruction. This study found 8.1% of cecal cancers presented as distal small bowel obstructions in elderly patients, often with advanced disease.
Area of Science:
- Gastroenterology
- Surgical Oncology
- Oncology
Background:
- Carcinoma of the cecum is the third most common malignancy of the large bowel.
- Obstruction is an infrequent presentation of cecal cancers.
Purpose of the Study:
- To examine cecal cancers that present with bowel obstruction.
- To determine the characteristics and outcomes of patients with obstructing cecal carcinoma.
Main Methods:
- Retrospective review of 136 patients with cecal carcinoma.
- Identification of cases presenting with distal small bowel obstruction.
Main Results:
- 11 out of 136 patients (8.1%) had obstructing cecal lesions presenting as distal small bowel obstructions.
- The mean age of these patients was 74 years.
- Surgical resection (right hemicolectomy with ileotransverse colostomy) was performed in all but one patient, with no operative mortality or significant morbidity.
Conclusions:
- Bowel obstruction due to cecal carcinoma is rare, typically occurring in elderly patients.
- This presentation is associated with advanced disease and a poor survival rate.
- Early diagnosis and treatment are crucial for improving outcomes in cecal cancer.
