Summary

Periorbital dermoid cysts, typically benign, can rarely extend into the skull. Two cases show delayed transcranial extension after initial apparent resolution, requiring complex surgical excision.

  • Two cases of lateral brow dermoid cysts initially recognized in newborns are presented.
  • These cysts appeared clinically resolved over time.
  • Later, they presented with significant bone erosion and intracranial extension.

Findings:

  • Delayed transcranial extension of periorbital dermoid cysts can occur.
  • This extension involves full-thickness bone erosion.
  • Combined extracranial and intracranial surgical approaches are necessary for excision.

Implications:

  • Highlights the potential for delayed complications of seemingly resolved dermoid cysts.
  • Emphasizes the need for long-term monitoring of periorbital lesions.
  • Informs surgical planning for complex orbitofacial tumor management.
