Summary

Human T-lymphotropic virus type 1 (HTLV-1) infection can cause tropical spastic paraparesis and intermediate uveitis. Early serological testing is recommended for patients with unknown uveitis and neurological symptoms from endemic areas.

Background:

  • Intermediate uveitis is inflammation of the middle layer of the eye.
  • Tropical spastic paraparesis (TSP) is a neurological disorder affecting the spinal cord.

Observation:

  • A 66-year-old woman presented with intermediate uveitis and progressive lower limb weakness.
  • Serological testing revealed infection with Human T-lymphotropic virus type 1 (HTLV-1).

Findings:

  • The patient was diagnosed with both TSP and HTLV-1-associated intermediate uveitis.
  • Treatment with oral steroids led to significant improvement in her condition.

Implications:

  • This case highlights the importance of considering HTLV-1 infection in patients with unexplained intermediate uveitis and neurological symptoms.
  • Serological screening for HTLV-1 is crucial in endemic regions or for individuals presenting with these combined symptoms.
