    Summary

    Electroconvulsive therapy (ECT) may increase syncope risk in older adults. Discontinuing ECT significantly reduced syncope events in a 79-year-old man with neurally mediated syncope.

    • Electroconvulsive therapy (ECT) is a vital treatment for severe depression.
    • Adverse effects of ECT include memory loss and potential fall risks.
    • Literature on ECT and falls is limited, primarily consisting of case reports.

    Observation:

    • A 79-year-old male patient diagnosed with neurally mediated syncope underwent maintenance ECT.
    • The patient experienced a notable increase in syncope frequency during ECT treatment.
    • Syncope episodes dramatically decreased after ECT was discontinued.

    Findings:

    • ECT treatment correlated with an increased frequency of neurally mediated syncope in an elderly patient.
    • Cessation of ECT led to a significant reduction in syncope episodes.
