Background:

Cataract surgery can lead to complications, including the development of iris epithelial inclusion cysts.
These cysts can cause visual impairment by obstructing the pupil.

Observation:

A 60-year-old male developed a pigmented epithelial inclusion cyst of the iris one year post-cataract surgery.
The cyst progressively enlarged, reducing vision to finger counting close to the face.

Findings:

Initial treatment with Neodymium-doped Yttrium Aluminium Garnet (Nd YAG) laser cystotomy was unsuccessful, with cyst recurrence.
En bloc excision of the cyst with sector iridectomy resulted in no recurrence at six-month follow-up.

Implications:

En bloc excision is a viable and effective surgical option for treating iris epithelial inclusion cysts.
This technique offers a potentially curative solution for vision-threatening iris cysts following cataract surgery.
