Summary

Patients with bilateral strokes often aspirate, with abnormal voluntary cough or gag reflex being key indicators. Additional clinical signs like dysphonia do not improve aspiration risk prediction in these stroke patients.

Area of Science:

  • Neurology
  • Swallowing Disorders
  • Clinical Diagnostics

Background:

  • Bilateral strokes can significantly impact swallowing function.
  • Aspiration is a common complication following stroke, leading to increased morbidity.

Purpose of the Study:

  • To identify clinical predictors of aspiration in patients with bilateral strokes.
  • To evaluate the efficacy of specific clinical signs in predicting aspiration risk.

Main Methods:

  • Seventy patients with bilateral strokes underwent neurologic and videofluoroscopic barium swallowing examinations.
  • Clinical signs including voluntary cough, gag reflex, and dysphonia were assessed.

Main Results:

  • Nearly half (48.6%) of patients aspirated.
  • Aspiration was associated with posterior circulation strokes, abnormal cough, abnormal gag, and dysphonia.
  • Abnormal voluntary cough or gag reflex were the strongest predictors of aspiration.
  • Additional clinical information did not enhance prediction accuracy.

Conclusions:

  • Abnormal voluntary cough and gag reflexes are crucial indicators for identifying aspiration risk in bilateral stroke patients.
  • Clinical assessment focusing on cough and gag reflexes can effectively identify patients at risk for aspiration.
