Laparoscopic heminephrectomy offers a safe and effective minimally invasive alternative to open surgery for duplex kidneys with poorly functioning upper poles. This technique demonstrates good outcomes with minimal complications in experienced hands.

Background:

  • Duplex kidneys with a poorly functioning upper pole moiety typically require ipsilateral upper pole heminephrectomy.
  • Open surgery has been the traditional approach, but advancements in ureteroscopy allow for minimally invasive alternatives.

Purpose of the Study:

  • To evaluate the safety and outcomes of laparoscopic heminephrectomy for duplex kidneys.
  • To assess the feasibility of this minimally invasive approach in a consecutive case series.

Main Methods:

  • A retrospective review of 14 patients who underwent unilateral laparoscopic heminephrectomy between February 2001 and May 2007.
  • Pre-operative imaging included ultrasonography, intravenous pyelography (IVP), and CT scans.
  • DMSA isotope scans were used to assess renal moiety function.

Main Results:

  • Mean operative time was 203 minutes.
  • No major intra-operative or early complications were observed.
  • Mean hospital stay was 4.1 days, with no significant symptoms or urinary tract infections reported during a 32-month follow-up.

Conclusions:

  • Laparoscopic heminephrectomy is a safe and valuable minimally invasive procedure for selected cases.
  • Meticulous manipulation of renal pedicle vasculature is crucial for preserving post-operative renal function.
