Background:

  • Phyllodes tumors are rare neoplasms typically found in the breast.
  • Prostate cancer is a common malignancy in men, often detected via prostate-specific antigen (PSA) screening.

Observation:

  • A 47-year-old male presented with hematospermia and significantly elevated serum PSA (60.76 ng/mL).
  • Needle biopsy showed atypical stromal cells; radical prostatectomy revealed a 2.9 cm phyllodes tumor.
  • Immunohistochemistry identified atypical stromal cells positive for vimentin, AR, ER, progesterone, and 5α-reductase.

Findings:

  • Genomic analysis detected a gain of 11p13 (WT1 gene) and losses of 1p36.23 and 12p12.1.
  • The elevated PSA levels were attributed to hyperplastic luminal cells potentially driven by 5α-reductase-positive stromal and epithelial cells.
  • Post-surgery, PSA levels normalized, with no recurrence or metastasis after 2 years.

Implications:

  • This case expands the known pathology of prostate tumors.
  • Understanding the hormonal and genetic drivers of such rare tumors is crucial for diagnosis and management.
  • Highlights the importance of comprehensive histopathological and molecular analysis in complex cases.
