Summary

Robotic thyroid surgery using the bilateral axillo-breast approach (BABA) is feasible and safe for early differentiated thyroid cancers and benign tumors. This initial experience shows promising results with no conversions to open surgery and minimal complications.

Purpose of the Study:

  • To report the initial two-year experience of a single surgeon using robotic thyroid operations with the BABA technique.
  • To evaluate the feasibility and safety of robotic thyroid surgery via the BABA approach.

Main Methods:

  • Prospective data collection from 93 patients undergoing robotic thyroid surgery (July 2008-July 2010) using the da Vinci-S system and BABA.
  • Evaluation of clinicopathological characteristics, operation types, times, results, hospital stays, and complications.

Main Results:

  • 93 robotic procedures performed, including total thyroidectomies, lobectomies, and one bilateral subtotal lobectomy.
  • No conversions to traditional endoscopic or open surgery.
  • Mean operation time: 288.5±48.0 min; mean hospital stay: 2.8±1.2 days.
  • Low complication rates: 3.2% transient hoarseness, 23.6% transient hypocalcemia (in total thyroidectomy patients), no permanent complications.

Conclusions:

  • Robotic thyroid surgery using the BABA approach is feasible and safe.
  • The procedure is effective for treating benign tumors and early differentiated thyroid carcinomas.
  • Initial results suggest a favorable safety profile with minimal complications.
