Background:

  • Body Mass Index (BMI) associations with health outcomes differ between Asian and European populations.
  • Asian populations exhibit increased risk for type 2 diabetes, hypertension, and hyperlipidemia at lower BMIs.
  • There is a recognized need for tailored surgical indications for Asian patients.

Framework:

  • The International Federation for the Surgery of Obesity and Metabolic Disorders – Asian Pacific Chapter (IFSO-APC) convened experts to establish consensus statements.
  • A voting system was employed to gauge expert agreement on proposed guidelines.
  • 95% of delegates affirmed the need for IFSO-APC consensus statements, and 98% supported new indications for Asian patients.

Implementation:

  • Bariatric surgery is recommended for obese Asian candidates (BMI ≥ 35) with or without comorbidities.
  • For Asian patients with type 2 diabetes (T2DM) or metabolic syndrome inadequately controlled by other means, bariatric/GI metabolic surgery is considered at BMI ≥ 30.
  • Surgical intervention is a potential alternative for suitable Asian candidates with BMI ≥ 27.5 experiencing inadequately controlled T2DM or metabolic syndrome.

Implications:

  • These consensus statements aim to guide the safe and effective advancement of bariatric and metabolic surgery in Asia.
  • The guidelines provide specific BMI criteria for surgical consideration in diverse Asian patient populations.
  • Establishing tailored indications promotes optimized treatment outcomes for obesity and related metabolic conditions in Asia.
