Acute Pyelonephritis II: Diagnostic Studies and Management

Introduction:For diagnosing acute pyelonephritis, a comprehensive patient history is collected to identify symptoms such as dysuria, frequent or urgent urination, flank pain, or costovertebral angle (CVA) tenderness that may suggest a kidney infection.Physical ExaminationDuring the physical examination, CVA tenderness is assessed. This involves gentle percussion over the costovertebral angle, where tenderness often indicates a kidney infection.Diagnostic TestsUrinalysis: Used to identify white...
Atypical Pneumonia01:14

Atypical Pneumonia

Atypical pneumonia, often caused by Mycoplasma pneumoniae, is a form of pulmonary infection that differs from the classical presentation of bacterial pneumonia in both its cause and clinical symptoms. Mycoplasma pneumoniae is a pleomorphic bacterium notable for its lack of a rigid cell wall. This structural characteristic imparts resistance to beta-lactam antibiotics and significantly influences the bacterium’s behavior within the human host.Other pathogens responsible for the disease include...

Pelvic actinomycosis, a rare infection often mimicking tumors, was diagnosed in a woman using imaging and histology. Long-term antibiotic treatment was initiated for this challenging pelvic actinomycosis case.

Area of Science:

  • Gynecology
  • Infectious Diseases
  • Medical Imaging

Background:

  • Pelvic actinomycosis is a rare bacterial infection caused by Actinomyces species.
  • It can present insidiously and mimic other pelvic pathologies, including malignancy.
  • Intrauterine devices (IUDs) are a known risk factor for pelvic actinomycosis.

Observation:

  • A 42-year-old female presented with a 5-week history of pelvic pain and a palpable 9 cm mass between her rectus muscles.
  • The patient had an intrauterine device (IUD) removed 3 weeks prior to presentation.
  • Diagnostic workup included ultrasound, magnetic resonance imaging (MRI), and histology.

Findings:

  • Imaging revealed a complex pelvic mass, raising suspicion for malignancy.
  • Histological examination and/or anaerobic culture confirmed the presence of Actinomyces granules, establishing the diagnosis of pelvic actinomycosis.

Related Experiment Videos

  • Actinomycosis can present as a challenging differential diagnosis in patients with pelvic masses.
  • Implications:

    • Early and accurate diagnosis of pelvic actinomycosis is crucial for appropriate management.
    • Definitive diagnosis relies on microbiological or histological evidence of Actinomyces.
    • Prolonged antibiotic therapy is the mainstay of treatment for pelvic actinomycosis.
