Summary

Mental challenge, like arithmetic, can significantly delay the onset of orthostatic syncope by boosting sympathetic activity. This finding offers a potential countermeasure for dizziness and hypotension.

Area of Science:

  • Cardiovascular Physiology
  • Neuroscience
  • Human Factors in Aerospace Medicine

Background:

  • Orthostatic vasovagal syncope is characterized by a sudden decline in sympathetic activity.
  • Mental challenges are known to activate the sympathetic nervous system.

Purpose of the Study:

  • To investigate if mental arithmetic can delay the onset of orthostatic syncope in healthy males.
  • To assess the impact of mental challenge on cardiovascular stability during orthostatic stress.

Main Methods:

  • Healthy male subjects underwent head-up tilt (HUT) combined with lower body negative pressure (LBNP) to determine orthostatic tolerance time (OTT).
  • A randomized crossover protocol compared HUT+LBNP alone versus HUT+LBNP with a mental arithmetic challenge prior to expected presyncope.

Main Results:

  • Mental challenge significantly improved orthostatic tolerance time in healthy male subjects.
  • The addition of mental loading delayed the onset of presyncopal symptoms during HUT+LBNP.

Conclusions:

  • Mental challenge serves as a viable countermeasure to enhance orthostatic tolerance.
  • This strategy may help alleviate orthostatic responses in individuals prone to dizziness, or during situations like hemodialysis and spaceflight.
