Summary

Observer variation in elbow arthrosis radiographic ratings was assessed. Fair agreement was found using the Broberg and Morrey system, with experience improving reliability. Binary systems showed moderate agreement.

Area of Science:

  • Orthopedic Surgery
  • Radiology
  • Medical Imaging

Background:

  • Elbow arthrosis is a degenerative joint disease.
  • Accurate radiographic assessment is crucial for diagnosis and treatment planning.
  • Observer variation can impact clinical decision-making.

Purpose of the Study:

  • To measure interobserver variability in radiographic grading of elbow arthrosis.
  • To evaluate the reliability of the Broberg and Morrey rating system.

Main Methods:

  • Thirty-seven orthopedic surgeons graded elbow arthrosis on 20 sets of radiographs.
  • The Broberg and Morrey rating system (grades 0-3) was used.
  • Interobserver reliability was assessed using the kappa multirater measure (κ).

Main Results:

  • Fair agreement was observed among surgeons using the Broberg and Morrey system.
  • Higher surgeon experience (years and elbow fracture volume) correlated with better agreement.
  • Simplified binary rating systems demonstrated moderate agreement.

Conclusions:

  • The Broberg and Morrey classification system exhibits only fair interobserver reliability for elbow arthrosis.
  • Surgeon experience and subspecialty influence rating reliability.
  • Binary rating systems may offer improved reliability for elbow arthrosis assessment.
