Background:

  • Monocular elevation deficiency (MED) is characterized by the inability to elevate the eye.
  • Myasthenia gravis (MG) can present with various ophthalmoplegia patterns, but mimicking MED is exceptionally rare.

Observation:

  • A 12-month-old girl exhibited left eye elevation deficit and blepharoptosis since 3 months of age.
  • Diagnostic tests included a weakly positive neostigmine test and negative repetitive nerve stimulation and acetylcholine receptor antibody assays.
  • Significant improvement in blepharoptosis was observed with the ice pack test.

Findings:

  • The clinical presentation mimicked MED, but standard MG tests were inconclusive.
  • The ice pack test provided a positive response, suggesting MG.

Implications:

  • Myasthenia gravis should be considered in the differential diagnosis of pediatric monocular elevation deficiency.
  • The ice pack test may be a valuable diagnostic tool for suspected myasthenia gravis presenting atypically in young children.
