PLOS mental health·2026

An adapted postdeployment intervention for UK armed forces personnel did not improve mental health outcomes. However, the intervention showed a modest reduction in binge drinking rates among participants.

Area of Science:

  • Military mental health research
  • Psychological interventions
  • Public health

Background:

  • Combat exposure is a known risk factor for psychological ill-health in armed forces (AF) personnel.
  • A U.S. psycho-educational intervention, Battlemind, demonstrated positive mental health effects in U.S. military personnel.
  • The effectiveness of an adapted, anglicized version of Battlemind for UK AF personnel required evaluation.

Purpose of the Study:

  • To assess the effectiveness of an anglicized Battlemind intervention compared to standard care.
  • To evaluate the intervention's impact on probable posttraumatic stress disorder and common mental disorders.
  • To examine the intervention's effect on alcohol misuse and binge drinking.

Main Methods:

  • A cluster randomized controlled trial was conducted with 2,443 UK AF personnel.

  • An anglicized Battlemind intervention was compared against the U.K. standard postdeployment brief.
  • Mental health was assessed using the Posttraumatic Stress Disorder Checklist (PCL-C) and General Health Questionnaire (GHQ-12).
  • Alcohol misuse and binge drinking were measured using the Alcohol Use Disorders Identification Test (AUDIT).
  • Main Results:

    • No significant difference in mental health outcomes or overall Alcohol Use Disorders Identification Test (AUDIT) scores was observed between groups.
    • Participants receiving the Battlemind intervention reported less binge drinking compared to the standard brief group (adjusted odds ratio = 0.73).
    • The observed effect on binge drinking was statistically significant but small in magnitude.

    Conclusions:

    • The anglicized Battlemind intervention did not improve mental health in UK armed forces personnel.
    • The intervention demonstrated a modest, positive impact on reducing binge drinking behaviors.
    • Targeted interventions to address alcohol misuse remain important for military populations.
