Background:

Brown-McLean syndrome is characterized by peripheral corneal edema.
Keratoconus is a progressive thinning of the cornea leading to irregular astigmatism.
The co-occurrence of these conditions is exceptionally rare.

Observation:

A 30-year-old female presented with decreased vision in her right eye.
She exhibited peripheral concentric corneal edema in the right eye, extending 2.5-3.5 mm from the limbus.
Ocular topography revealed bilateral keratoconus.

Findings:

The patient had unilateral Brown-McLean syndrome and bilateral keratoconus.
No history of ocular surgery, trauma, or systemic illness was reported.
This represents the first reported instance of this specific combination of corneal conditions.

Implications:

This case expands the understanding of potential co-existing corneal pathologies.
It suggests the need for comprehensive corneal evaluation, including topography, in patients presenting with atypical corneal edema.
Further research may explore potential underlying mechanisms or genetic links for this rare association.
