Summary

Robotic thymectomy using the da Vinci system is safe and effective for anterior mediastinal diseases. Experience improved efficiency, reducing operative time and hospital stay with no complications.

Purpose of the Study:

  • To describe a 3-year experience with robotic thymectomy using the da Vinci Surgical System.
  • To evaluate the safety and efficacy of robotic thymectomy for diverse pathologies.

Main Methods:

  • Nine patients underwent totally endoscopic robotic thymectomy for myasthenia gravis, mediastinal mass, thymoma, or hyperparathyroidism.
  • Patients were divided into early (Group A) and later (Group B) experience cohorts.
  • Data analysis included Fisher exact and Mann-Whitney tests.

Main Results:

  • Complete robotic resection of the thymus was achieved in all patients.
  • Later experience (Group B) showed significant reductions in operative time and a trend toward decreased chest tube days and length of stay.
  • No morbidity or mortality was observed.

Conclusions:

  • Robotic thymectomy is a safe and effective minimally invasive approach.
  • The learning curve for robotic thymectomy facilitates surgeon adoption for mediastinal procedures.
