Minimized cardiopulmonary bypass (mini-CPB) systems, with or without Bioline-coating, demonstrated superior myocardial preservation compared to conventional cardiopulmonary bypass (CCPB). However, no significant differences were observed in inflammatory response or early clinical outcomes.

  • Patients were allocated to three groups: Bioline-coated mini-CPB, uncoated mini-CPB, or CCPB.
  • Measurements included cytokines, myocardial markers (CK, CK-MB, troponin-T), hematocrit, and platelet counts up to 48 hours postoperatively; clinical outcomes assessed at 3 months.
  • Main Results:

    • Both mini-CPB groups showed significantly reduced myocardial markers (CK-MB, troponin-T) compared to CCPB.
    • No significant differences in inflammatory markers (interleukin-6, -8, TNF-alpha) were found among the groups.
    • Extracorporeal circulation and cross-clamp times were longer in mini-CPB groups, but clinical outcomes, hematologic parameters, and inflammatory responses were comparable.

    Conclusions:

    • Minimized cardiopulmonary bypass systems, irrespective of Bioline-coating, offer improved myocardial protection.
    • Mini-CPB did not significantly alter the inflammatory response, hematologic effects, or early clinical outcomes compared to CCPB.
    • Further research may be needed to elucidate the long-term benefits and optimal application of mini-CPB technologies.
