Background:

  • Accurate estimation of left atrial (LA) pump function is vital for cardiac patient management.
  • Transthoracic echocardiography is a common imaging modality in cardiology.

Purpose of the Study:

  • To evaluate mitral annular late diastolic velocity (A') as a predictor of LA pump function.
  • To determine the role of A' in patients undergoing paroxysmal atrial fibrillation (AF) ablation.

Main Methods:

  • One hundred and four patients scheduled for paroxysmal AF ablation were studied.
  • Echocardiography and multi-detector computed tomography (MDCT) were performed prior to ablation.
  • LA emptying fraction (LAEF) from MDCT was correlated with echocardiographic parameters, including A'.

Main Results:

  • Mitral annular late diastolic velocity (A') showed a significant correlation with LA emptying fraction (LAEF) (r=0.59, P<0.0001).
  • A cutoff value of 7.4 cm/s for A' accurately identified impaired LA pump performance (LAEF <30%) with 93% sensitivity and 81% specificity.
  • The negative predictive value of A' for impaired LA pump function was 99%.

Conclusions:

  • Mitral annular late diastolic velocity (A') is a simple, non-invasive, and reliable echocardiographic parameter for estimating LA pump function.
  • A' is particularly useful in patients with paroxysmal AF.
  • This method aids in the clinical management of cardiac patients.
