Insights

This study reports a rare case of extensive multiple coronary cameral fistulae in an elderly woman. The condition presented with myocardial hypoperfusion, requiring detailed cardiac evaluation.

Area of Science:

  • Cardiology
  • Cardiovascular Imaging
  • Interventional Cardiology

Background:

  • Coronary artery fistulae are uncommon congenital or acquired abnormalities.
  • Pre-operative cardiac evaluation is crucial for patients with cardiovascular risk factors.

Observation:

  • An 82-year-old woman with hypertension and diabetes underwent cardiac evaluation.
  • Myocardial perfusion imaging showed hypoperfusion in the apicoseptal and inferior segments.

Findings:

  • Coronary angiography revealed extensive multiple coronary cameral fistulae.
  • These fistulae drained homogeneously and circularly into the left ventricle.

Implications:

  • Coronary cameral fistulae can lead to myocardial ischemia despite absence of obstructive coronary artery disease.
  • Accurate diagnosis through angiography is essential for appropriate management.
  • This case highlights the importance of considering rare coronary anomalies in cardiac evaluations.

Coronary Circulation

The heart, an organ critical to survival, gets nourishment not from the blood it pumps but from a separate circulation system known as coronary circulation. This is the shortest circulation in the body and is responsible for supplying the heart with the nutrients it needs to function effectively.
Coronary circulation begins at the base of the aorta, where two main arteries arise—the left and right coronary arteries. These arteries encircle the heart in the coronary sulcus and supply the...

Coronary Artery Disease II: Pathophysiology

Coronary Artery Disease (CAD) originates from a series of events that impair the function of coronary arteries, the blood vessels responsible for delivering oxygen-rich blood to the heart muscle. The pathophysiology of CAD is closely linked to atherosclerosis, a chronic inflammatory and lipid-driven condition affecting the vascular endothelium.1. Endothelial DamageThe process begins with damage to the vascular endothelium, which serves as a protective barrier between the blood and the vessel...

Imaging Studies for Cardiovascular System V: CT

Cardiac computed tomography (CT) scanning is an advanced cardiac imaging technique that utilizes CT technology, with or without intravenous (IV) contrast, to produce accurate cross-sectional virtual slices of specific areas of the heart, coronary circulation, and major blood vessels such as the aorta, pulmonary veins, and arteries. The computer processes these slices to generate three-dimensional images. Multidetector CT (MDCT) is a rapid form of CT scanning that captures multiple slices...
