Background:

  • Primary breast sarcoma is a rare malignancy.
  • Distinguishing it from more common breast cancers like infiltrating ductal carcinoma is crucial for appropriate management.
  • Imaging plays a key role in the initial detection and characterization of breast masses.

Purpose of the Study:

  • To delineate the characteristic imaging findings of primary breast sarcoma.
  • To assist radiologists in identifying potential cases of primary breast sarcoma on mammography, sonography, and MRI.

Main Methods:

  • Retrospective review of imaging studies (mammography, sonography, MRI) in patients with histopathologically confirmed primary breast sarcoma.
  • Exclusion of malignant phyllodes tumors.
  • Application of the American College of Radiology Breast Imaging Reporting and Data System (BI-RADS) lexicon for standardized assessment.

Main Results:

  • Twenty-four female patients with a mean age of 56 years were analyzed.
  • Mammography typically showed noncalcified oval masses with indistinct margins.
  • Sonography frequently revealed oval, solid, hypoechoic, and hypervascular masses with indistinct margins and posterior acoustic enhancement.
  • MRI demonstrated T2-hyperintense masses with irregular margins and inhomogeneous enhancement.

Conclusions:

  • Primary breast sarcoma exhibits distinct imaging features that differ from infiltrating ductal carcinoma.
  • The presence of a large, oval, hypervascular mass with indistinct margins on imaging should increase suspicion for primary breast sarcoma.
  • Prompt biopsy is recommended when these suspicious imaging findings are observed.
