Women with pelvic organ prolapse (POP) experience higher rates of constipation symptoms, particularly outlet constipation. Current diagnostic criteria may not fully capture these functional bowel disorders in advanced POP cases.

Background:

  • Pelvic organ prolapse (POP) is a common condition affecting women's pelvic floor support.
  • Functional bowel disorders (FBDs) are prevalent, but their association with POP requires further investigation.

Purpose of the Study:

  • To compare the frequency of constipation and other FBDs in patients with and without POP.
  • To assess the utility of the Rome II criteria in classifying FBDs in POP patients.

Main Methods:

  • A case-control study involving 128 patients with stage 3-4 POP and 127 controls with stage 0-1 support.
  • FBDs were assessed using the Rome II Modular Questionnaire and predefined defecatory disorders.

Main Results:

  • Women with POP showed significantly higher odds of reporting symptoms of outlet constipation, including straining and incomplete emptying.
  • While functional constipation and IBS-constipation rates were similar, POP cases were more likely to meet criteria for IBS-any type.
  • POP patients reported increased abdominal discomfort and more frequent bowel movements.

Conclusions:

  • Patients with POP exhibit a greater likelihood of experiencing outlet constipation and other FBDs.
  • The Rome II criteria may be insufficient for accurately classifying FBDs in individuals with advanced POP.
