Summary

Primary intraocular lymphoma can affect young adults, presenting as chronic panuveitis. Prompt diagnosis via vitrectomy and treatment with intravitreal methotrexate led to a favorable outcome in this case.

Anterior High-Resolution Optical Coherence Tomography in the Diagnosis and Therapeutic Monitoring of Ocular Surface Squamous Neoplasia

Background:

  • Primary intraocular lymphoma (IOL) is a rare extranodal non-Hodgkin lymphoma.
  • It often presents as a chronic, treatment-resistant panuveitis, mimicking other inflammatory conditions.

Observation:

  • A 33-year-old male presented with a two-year history of bilateral blurred vision.
  • Ocular examination revealed significant findings including keratic precipitates, anterior chamber reaction, posterior subcapsular cataracts, and vitreous infiltration.

Findings:

  • Diagnostic 25-gauge pars plana vitrectomy with cytologic evaluation confirmed intraocular lymphoma.
  • The patient received intravitreal methotrexate for both eyes, resulting in a positive clinical response.
  • Central nervous system evaluation for systemic lymphoma was negative.

Implications:

  • This case highlights the importance of considering primary intraocular lymphoma in young adults with chronic, recalcitrant panuveitis.
  • Early diagnosis and targeted treatment, such as intravitreal methotrexate, can lead to favorable visual outcomes.
  • Further research into the diagnostic and therapeutic strategies for IOL is warranted.
