Stroke and transient ischemic attack (TIA) patients show similar depression rates post-hospitalization. A significant number of patients with persistent depression remained untreated, highlighting a gap in care for both stroke and TIA survivors.

Purpose of the Study:

  • To compare depression rates and antidepressant use between patients with ischemic stroke and TIA.
  • To identify factors associated with persistent depression in stroke patients.

Main Methods:

  • Secondary analysis of a prospective US cohort registry (2006-2008).
  • 1450 ischemic stroke and 397 TIA patients assessed for depression (PHQ-8 score ≥ 10) and medication use at 3 and 12 months post-hospitalization.

Main Results:

  • Similar depression frequencies at 3 months (17.9% stroke vs. 14.3% TIA) and 12 months (16.4% stroke vs. 12.8% TIA).
  • Comparable rates of newly identified depression between 3 and 12 months (8.7% vs. 6.2%).
  • High rates of untreated persistent depression in both groups (67.9% stroke, 70.0% TIA).

Conclusions:

  • Stroke and TIA patients experience similar depression frequencies and onset patterns post-hospitalization.
  • A substantial proportion of patients with persistent depression after stroke or TIA do not receive antidepressant treatment.
