Area of Science:

  • Hepatobiliary Surgery
  • Surgical Oncology
  • Gastroenterology

Background:

  • Transverse hepatectomy, involving removal of hepatic segments IVB, V, VI, and III, was introduced in 1990.
  • The first Hungarian transverse hepatectomy was performed in 1995.
  • This study reviews a decade of experience with this surgical technique.

Purpose of the Study:

  • To summarize the experience with transverse hepatectomies from 1995 to 2008.
  • To evaluate the safety and efficacy of transverse hepatectomy.
  • To assess the utility of transverse hepatectomy in metastasis surgery.

Main Methods:

  • A retrospective review of 72 trisegmentectomies was conducted.
  • Twenty-two of these procedures were transverse hepatectomies.

  • Data on resection time, blood transfusion, exclusion time, hospital stay, and mortality were analyzed.
  • Main Results:

    • Average resection time was 1 hour 20 minutes.
    • Average blood transfusion requirement was 0.8 units per patient.
    • Average postoperative hospital stay was 10.1 days, with 2 perioperative deaths.

    Conclusions:

    • Transverse hepatectomy is a bloodless and relatively easy procedure.
    • It allows for safe removal of antero-lateral liver segments.
    • The technique is highly useful in metastasis surgery with minimal morbidity and mortality.
