Background:

  • Cutaneous leishmaniasis (CL) is a parasitic disease primarily affecting exposed skin, often on limbs.
  • Leishmania species, particularly those from the Viannia subgenus, cause CL and can present diagnostic and therapeutic challenges.

Observation:

  • A rare case of CL occurred on the fourth finger of the left hand in a female patient with occupational exposure in a forested area.
  • The patient presented with a characteristic ulcer with infiltrated borders, indicative of CL.

Findings:

  • Diagnosis confirmed CL caused by Leishmania (Viannia).
  • Initial treatment with antimony was ineffective.
  • Combination therapy with antimony and pentoxifylline resulted in clinical cure.

Implications:

  • This case underscores the unusual periungual (around the nail) presentation of CL.
  • It highlights the potential for pentoxifylline to enhance treatment efficacy in refractory CL cases.
  • The findings suggest a need for considering alternative therapeutic strategies for difficult-to-treat CL presentations.
